The request form asks for your name, email address, phone number, ZIP code, desired timing and a description of the drainage problem. If photo upload is added, images and their embedded metadata may also be collected. Do not submit sensitive financial, medical or identity-document information.

Retention and security
Information should be retained only as long as reasonably needed for referral operations, legal obligations, dispute resolution and security. No internet system is perfectly secure. The live operator must configure access controls, encrypted transport and an appropriate deletion process before accepting production requests.
Your choices
You may decline to submit the form and contact providers directly. A production version should publish an operator-controlled privacy contact for access, correction or deletion requests before intake is enabled.
